Supplemental Info Request
Once we receive your contact information from AMCAS, an electronic supplemental application will be emailed to you. The supplemental information requested will include the following with a deadline of November 1st.
- Supplemental application which includes:
- A non-refundable filing fee of $65.
- Joint MD/PhD educational track information.
- Graduate degree enrollment information.
- Oklahoma ties documentation.
- Letters of recommendation. We will be using the AMCAS Letters program this year and all letters should come from AMCAS. You may either send a premedical committee letter and 1 faculty letter or three faculty letters. Our definition of a faculty letter is one written by a college/university faculty member who has instructed you at the college/university level.
