Appointment Information

Appointments can be made by calling 271-4311. You may leave a voicemail if the wait is prolonged and someone will call you back within 24-48 hours. Another new convenient way to make an appointment is through our web portal for patient communication. Patients and potential patients have online access to information about becoming a patient, they can also request an appointment or perform any other task they might otherwise do by telephone. Phone Calls for medical information:

Our goal is to return all patient phone calls within 24 hours or one working day.

Late for appointments and no-shows:

It is very important that you report to the clinic on time for your appointment. New patients must come 30 minutes prior to their appointment to complete necessary paper work. Established patients need to come 15 minutes early. If you are 15 minutes late for your appointment, we may ask you to reschedule your appointement or we may work you in when there is an opening. If, for any reason, you are unable to make it for your appointment, please contact the clinic to cancel and reschedule your appointment. Your cancelled appointment can be used for another patient. A missed appointment means a lost opportunity for another patient who may need that appointment time.

Same Day Appointments:

We have dedicated several appointment slots in the Green Clinic for patients with acute medical needs who cannot wait for an opening in their PCP's schedule. Patients being seen in this clinic will commonly be seen by one of our Physician Assistant faculty. If your health care requires a more extended visit, we recommend you make an appointment with your primary care physician in Rose, Blue or Purple Clinic.

Referral Appointment Information:

If your physician has requested a referral for you, our Referral Office will process the referral for you. We will contact your insurance company for an authorization, schedule the appointment, and notify you by mail of your appointment date, time and location. If you have not recieved the notification by mail within 10 working days, please contact the referral office at405-271-4287. In case of an urgent appointment need, your physician's nurse will contact you by telephone. Please make sure your mailing address and telephone number is updated at your clinic visit so that your appointment information reaches you in a timely manner.

General Information

Payment Policy:

Payment is expected at time of service unless you are covered by insurance. Co-payment and non-covered charges are expected at the time of service. It is your responsibility to notify the staff if you have changed your address, telephone number, or insurance coverage since your last visit.

Medicine Refills:

Always ask your physician for refills of your medication during your appointment. If you need medicine refills after your appointment, please call your pharmacy. Ask them to fax your requests to 405-271-2782.  PLEASE DO NOT wait until you are out of medications before calling your pharmacy. Please allow at least 2 working days for processing your refill requests.

Radiology (X-ray copies):

Copies of your x-rays will be provided to a medical consultant for your continuous care. Our clinic is equipped with a digital x-ray; therefore, copies will be provided to you on a CD. Please allow 48 hours to process your request. Copies of your older x-rays will be provided via films. It will take 5 working days to process your request. If you want to keep a copy of your x-rays for personal reasons (whether new or older films), there is a nominal charge of $5.00 per copy. Please call 271-4311 ext. 31102 to make sure your copies are ready for pickup before you make a trip to the clinic. 

Medical Records Copies:

Copies of your records will be sent to your consulting physician free of charge. However, if you need a copy of your record for your personal file, there is a charge of $1.00 for the first page and $0.50 for each additional page. Payment is due at the time of pickup. A signed authorization form may be required before releasing the patient records. Please allow 5 working days to process your request. 

Laboratory Test Results:

The lab test(s) requested by your physician will be sent to an outside lab for analysis. The results of your test(s) will be available in 7 to 14 days and will be mailed to you. Clinic and/or nursing staff are not authorized to give any test results over the phone. If you have not recieved your test results by the end of the 3 weeks, please contact the clinic to verify that we have your correct mailing address. If there is a critically abnormal result, your physician or nurse will contact your immediately.
