The Children's Patient Access Center will schedule an appointment and notify your office of the time and date within 72 hours. In order to process the appointment, please include the following information.
- Legible copy of Insurance card(s) (front and back)
- Office note pertaining to this referral
- If required, copy of referral or authorization
- Lab results pertaining to this referral
