 New Patient Information

Please bring the following with you on your first visit:

  • Social Security Number
  • Insurance Information
    If you have an insurance card, bring it. To bill your insurance carrier(s) for services rendered, we will need the name of the carrier (company), their complete mailing address, your policy's group number and your personal identification number. If your insurance carrier requires a referral form, please bring it with you to your first appointment. If you are covered by Medicare or Medicaid, please bring the appropriate card as well. You may also be asked to pay for services not covered by insurance. Please bring cash, check or major credit card (Visa, MasterCard or American Express).
  • Records 
    If your visit is for a second opinion or on a physician's referral, you may need to bring medical records, x-rays, results from lab tests, pathology slides, or other materials. Your physician may have sent these to us already. Be sure to ask your doctor before your visit here.
  • All Medications in Original Container
    You are required to bring ALL medications to your appointment, prescription medications as well as over the counter medications.
  • Advance Directive
    An Advance Directive is a written document you prepare stating how you want medical decisions made if you lose the ability to make decisions for yourself. If you have an Advance Directive (a Living Will and/or Durable Power of Attorney for Healthcare), please bring it with you. If you do not have one, we encourage you to take the time to review and complete The Oklahoma Advance Directive for Health Care.
  • Insurance
    Just as each individual is different, so too are the vast number of health care plans. OU Physicians is contracted with a wide range of HMOs, PPOs, and other health care plans.

Q: What kind of identification is needed?
A driver's license or employee ID will serve as proof of ID.

Q: Do you take my insurance?
OU Physicians accepts many insurance plans, but you may wish to call 918-619-4400 to verify your plan with us.

Q: What if I have no insurance? 
If you don’t have insurance coverage, you may receive a cash discount if all charges due that day are paid in full.  Payment arrangements may be made on an individual basis.

Q: What kind of payment is required?
Services received are to be paid for at the time of service. If you have insurance, please bring your insurance card and any copay amounts that are due from you.
