 Insurance Information

Understanding your insurance can often be a confusing task, but when visiting your healthcare provider, it is important to find out if the services you need will be covered by your insurance policy. Also, you will want to be prepared if your insurance policy requires an office co-payment. Contacting your insurance company is the best way to get the most updated information on your policy. OU Physicians in Tulsa accepts most insurances. For questions, call ​(918) 619-4400. 

Billing Information

Patients are given the option to complete payment online at https://www.paymydoctor.com/ or by calling (918) 619-4400 to make a payment.

Medical Records Release

If you are interested in obtaining a copies of your health information, you have the option to access your records online via OU Medicine’s patient portals. (we need to make sure to add Tulsa to this!) If you’d like to pick up hospital records in person, please download, print and complete the form below. Please check as many items needed and be sure to include encounter dates.

You may fax, mail, or personally deliver your completed form to the Health Information Management Department. We will need a valid photo ID (driver’s license, military ID or state ID) and a telephone number.
